Driver Abstracts

Driver’s Abstracts are easiest to obtain when you can come into the office personally with acceptable ID. If that isn’t an option there are two alternatives. First you can have someone known to you come in on your behalf with a Driver Abstract Consent form filled out by you, then they will have to fill out a Driver Abstract Statement of Intent as well while in the office. Once that is done they will need to pay the fee and we will give them your Abstract. It’s up to them to get the abstract to you. The second option is to fill out the Notarized Request for Personal Driving and Motor Vehicle Information Form. This needs to be filled out, Notarized, then the original must be mailed to our office with a check or money order for the service amount including a stamped and addressed return envelope as we will need to mail your Abstract back to you.
